ATTACK SPORTS PARTICIPATION AGREEMENT
TERMS and CONDITIONS
Effective Date: January 1, 2026
By registering for any Attack Sports programs, camps, tournaments, leagues, training sessions, or activities (collectively, “Programs”), you automatically agree to these Terms and Conditions, as well as all relevant liability waivers provided for each program. Each program’s waiver contains additional, program-specific details regarding risks, responsibilities, and safety protocols, which are also binding upon registration.

1. ACKNOWLEDGMENT OF RISK
Participation in Attack Sports Programs involves inherent risks, including but not limited to:
- Physical injury (bruises, sprains, fractures, concussions, dislocations, head/neck injuries, paralysis)
- Emotional injury
- Property damage
- Death
Risks include, without limitation:
- Use of ice, synthetic ice, training devices, sticks, pucks, nets, and other hockey equipment
- Off-ice strength, conditioning, agility, and fitness exercises
- Coaching, supervision, and instruction by staff and volunteers
- Travel to and from external arenas or facilities
- Equipment malfunction or improper maintenance
- Participation in competitions, tournaments, or games
- Participation is voluntary. Participants assume all risks on behalf of themselves and any minors they are signing for.
2. RELEASE, WAIVER and INDEMNITY
On behalf of yourself and any minors you are signing for (collectively, “Participants”), you release, waive, and discharge:
Attack Sports Inc., its directors, officers, shareholders, employees, volunteers, agents, partners, affiliates, landlords (including Petwin Fairview Corp. and/or any other facility or arena used by Attack Sports), and all related entities (“Released Parties”) from any claims, demands, or causes of action, including but not limited to claims arising from negligence or acts/omissions of the Released Parties, related to participation in Programs, including:
- Use of Attack Sports facilities or equipment
- Participation at external arenas, ice rinks, or alternate locations used by Attack Sports
- Attendance at tournaments, games, or off-site events
- Interaction with staff, coaches, volunteers, or other participants
Participants also agree to indemnify and hold harmless the Released Parties from claims arising from their own actions, including injury, death, or property damage.
3. MEDICAL AUTHORIZATION and INJURY POLICY
- I authorize Attack Sports staff or volunteers to arrange medical treatment or transport in emergencies
- I agree to bear all costs not covered by insurance
- I confirm that I and/or any minor participant have adequate insurance
Injury and Compensation:
No compensation, refund, and/or credits will be provided for injuries sustained during Attack Sports Programs
4. CONDUCT, RULES and MISREPRESENTATION
Behavior:
- Participants and parents must follow all rules, instructions, and safety protocols
- Violent, threatening, abusive, or unsafe behavior may result in removal from Programs without refund and/or credit
Misrepresentation:
- Teams and/or athletes providing false information about skill level, age, or division without prior approval will be removed without refund and/or credit
Team Registration by Managers/Coaches:
- Team managers/coaches confirm that all participants and guardians have consented
- Attack Sports is not liable if consent was not obtained
5. EQUIPMENT and ATTIRE
- Athletic clothing and clean, dry indoor shoes are required for off-ice activities in our training facility
- Required and approved hockey equipment (helmet, gloves, skates, neck guards, pads, etc.) must be worn for on-ice sessions
- Attack Sports is not responsible for lost, stolen, and/or damaged to personal property
Coaching Disclaimer:
- Coaches and instructors are not liable for injuries and/or damages caused by improper execution of instructions
- Participants must follow guidance and use equipment safely
6. EXTERNAL FACILITY and ARENA USE
- Programs may be held at external arenas/facilities
- Participants/teams are financially responsible for damages
- Failure to follow facility rules may result in removal without refund and/or credit
7. PHOTOGRAPHY, VIDEO and MEDIA
- Attack Sports may photograph and/or video participants for marketing, education, and/or internal use
- Participants consent to the use of these images/footage without compensation
- Parents/participants may not record or redistribute without written consent from Attack Sports
8. FORCE MAJEURE / GOVERNMENT RESTRICTIONS
Attack Sports is not liable for cancellations, postponements, or alterations caused by unforeseen circumstances, including, but limited to:
- Facility closures, power outages, weather, ice conditions
- Government restrictions or public health measures
- Pandemics, natural disasters, or other events outside our control
10. GOVERNING LAW and JURISDICTION
- These Terms and Conditions are governed by the laws of Alberta, Canada
- Any disputes are resolved exclusively in Alberta courts
- If any provision is unenforceable, the remainder remains in full force
11. ACKNOWLEDGEMENT
By registering in any Attack Sports Programming, you confirm:
- You have read, understood, and agree to these Terms and Conditions
- You voluntarily assume all risks and responsibilities for participation
- You agree to abide by all rules, codes of conduct, and safety protocols
- You consent to the waiver and release provisions for yourself or any minor participant
